Moorfields Eye Hospital NHS FT: Outline Business Case for EPR Procurement

Background

Moorfields Eye Hospital NHS Foundation Trust provides a wide range of ophthalmic services, caring for patients with routine medical needs as well as those with rare and complex conditions. It serves the NHS and private sectors in the UK and delivers care through its international services. In partnership with the UCL Institute of Ophthalmology and other strategic partners, it conducts world-leading research and plays a leading role in the training and education of eye care clinicians.

Requirement

Moorfields Eye Hospital identified the constraints of operating without the support of an Electronic Patient Records (EPR) system and was designated as a priority Trust for levelling up digital maturity within the NHS.

To drive forward the procurement of an EPR, Moorfields turned to Ideal Health to work with the Trust in:

  • identifying and articulating their functional and non-functional requirements within an Output Based Specification; and
  • developing a HMG Green Book-compliant business case in support of the required investment.

Complication

As a world-leading Ophthalmology service, Moorfields Eye Hospital has made significant investment in the development of OpenEyes as their specialist Ophthalmology clinical noting system. OpenEyes has class-leading functionality which has won it a very high level of clinical support and commitment.  Through the extensive and committed use of OpenEyes, a valuable pool of clinically rich data has been accumulated which forms the foundation of the Trust’s Ophthalmology research and long-term data re-use.

The Trust wished to review and assess the options of retaining OpenEyes as their specialist clinical noting solution versus replacing OpenEyes within the context of an ophthalmology module within the EPR.

Approach and Solution

Ideal Health created a team which blended specific skills required to develop the business case (such as expertise in the application of mandated economic modelling) with experience in analysing and documenting requirements in a form that resulted in a modern Output Based Specification. We also provided dedicated project management support to assist with corralling the inputs form across the Trust for the requirements, business case and wider procurement preparation.

A Clinical and Operational Working Group was established with the remit to validate and agree the requirements and inputs required for the business case (e.g. the benefits model)

Structured interviews and workshops were held with clinical, managerial, and administrative staff based on a core set of EPR requirements built up by Ideal Health as a library over many similar engagements. Each requirement area was shaped and honed to reflect the specific and unique needs of Moorfields resulting in a set of requirements that the organisation felt confident in saying they ‘owned’.

Details from the interviews and workshops also fed into the development of the business case.  Market interest and indicative supplier costs were elicited via a pre-market engagement exercise, and a benefit and cost model built in the form of a Comprehensive Investment Appraisal Model.

Once produced, Ideal Health led the navigation of internal and external assurance for the business case including accommodating ICB, NHS Frontline Digitisation and EPR Investment Board requirements.

Outcome and Benefits

The Outline Business Case received EPRIB approval which allowed Moorfields to proceed to procurement supported by our Output Based Specification.

Ideal Health remains engaged with Moorfields and is supporting the procurement and further development of the Full Business Case.
